Tesla data confirms Utah crash details, NHTSA investigating

On more than a dozen occasions, the auto is said to have registered a total absence of hands on its wheel, and twice for periods exceeding a minute in length.

The driver had turned on the "autosteer" and "cruise control" features about 80 seconds prior to the crash and taken her hands off the wheel, Tesla officials were quoted as saying to police on Wednesday by the Guardian. Federal officials blamed the truck's driver for failing to yield, on the driver for relying too heavily on Autopilot, and on Elon Musk-led Tesla - the National Transportation Safety Board ruled that Autopilot contributed to the crash by allowing lengthy disengagement from the driving process, on a roadway unsuitable for the semi-autonomous-driving system.

"This is consistent with her admission that she was looking at her phone at the time", police said.

The two agencies also continue to investigate a March crash of a Tesla Model X in Mountain View, Calif.

The police said the Tesla Model S during that day's trip registered more than a dozen instances of the driver's hands being off the steering wheel.

The woman suffered a broken foot and some abrasions from the air bag deployment but, according to police, escaped relatively unscathed from the crash that demolished the front end of the Tesla.

A Tesla sedan with a semi-autonomous Autopilot feature rear-ended a fire department truck at 60 miles per hour (97 kph) apparently without braking before impact on May 11, 2018, but police say it's unknown if the Autopilot feature was engaged.

"About 1 minute and 22 seconds before the crash, she re-enabled Autosteer and Cruise Control, and then, within seconds, took her hands off the steering wheel again", the police report says.

A 28-year-old Lehi woman was issued a citation after her Tesla slammed into the back of truck.

The Tesla report also noted that instructions provided to drivers about their vehicle's automation features expressly instruct that they maintain attention, and hands on the wheel, at all times. "Based upon witness information, the driver of the Tesla did not brake or take any action to avoid the collision", a statement from South Jordan Police said. It is also probing the January crash of a Tesla vehicle apparently traveling in Autopilot that struck a fire truck.

It's unclear if that system activated before the Utah crash. She manually pressed on the brake just before the crash. After a fatal Tesla Model X crash on Highway 101 in Mountain View in March, Tesla reported that the driver of that auto had been warned repeatedly during the trip to put his hands on the wheel, and that his hands were not detected on the wheel for six seconds before the collision.
